Mirabella Portland (MBP) is committed to providing employees with a highly competitive and comprehensive benefits program. Details on benefits are available from the Human Resources Department.

Flexible Schedules - Work with your supervisor to find the schedule that best suits your needs.  You can move back and forth depending on shift availability between on call, part-time and full time employment.

On-call Employees - employees who work a minimum of 48 hours per month which includes 2 weekend shifts per month based on department need.

Part-time Employees - employees who work at least one shift on a regular schedule.

Full-time Employees - employees who scheduled to work 40 hours a week

ALL EMPLOYEES ARE ELIGIBLE FOR:

During employee hours, employees are welcome to take advantage of the Wellness Center.  The Wellness Center has a sauna, steam room, workout equipment and indoor UV pool.   On Sundays employees may bring immediate family members to the Wellness Center to use the pool.

Employee Meals
Mirabella Portland has wonderful meals that employees can purchase at a discount.

On call employees who have completed one year of employment and work on a holiday will receive holiday pay.  On call employees have no set schedule and agree to work

Transportation
Through Tri-Met, Mirabella Portland will provide bus passes at discounted rates for employees.

ALL PART-TIME AND FULL-TIME EMPLOYEES ARE ELIGIBLE FOR:

Holiday Pay
Effective after 90 days, you will be eligible for six paid holidays.  Holiday pay is paid based on the number of hours you work up to 8 hours. 

Floating Holiday
There is an additional floating holiday after completing one year of continuous active service. Floating Holiday pay is paid based on the number of hours you work up to 8 hours.

Vacation
Enjoy this wonderful paid time off benefit.  After completing 1-4 yrs - 2 weeks; after completing 4-9 yrs - 3 weeks; after completing 10+ yrs - 4 weeks.  Vacation pay is paid based on the number of hours you work.

Sick Leave
After you successfully complete the trial service period, sick leave will accrue at the rate of 8 hours a month for full time employees.  Part-time employees will accrue based on number of hours worked up to 8 hours a month. 

Paid Jury Duty
Full-time and part-time employees who are called for jury duty are encouraged to meet this civic obligation.  Employees will be paid their regular wage for scheduled work missed because of jury duty.

403(b) Plan
Start your 403(b) upon hire.  You can choose between pre or post tax accounts.  Choose between Ameriprise or Valic.

Matching 403(b)
After one calendar year of employment a 3% match is available for your pre- or post- tax 403(b) accounts. Choose between Ameriprise or Valic.

Pension
After completing 12 consecutive months of employment, if over the age of 21, worked at least 1000 hours you will automatically be enrolled in the pension plan which has a 3% annual contribution.

Employees who work at least thirty hours a week are eligible for medical, dental, life insurance and supplemental insurance benefits. 

Kaiser Medical - HMO
Primary office visit $15.00 co pay. Specialist office visit $25.00 co pay. Vision office visit $15.00 co pay.  Urgent care visit $15.00 at a Kaiser facility.  The deductible only applies for hospital inpatient surgery, outpatient surgery, outpatient rehabilitation, emergency room visits. There is a $250 annual deductible with an out of the pocket maximum of $2000.00.   Pharmacy co-payments are $15, $30 or with mail maintenance prescriptions available at 2 co pays for a 90 day supply. 

Kaiser Medical-Added Choice
Depending on your needs, choose between Kaiser, the FirstHealth Network PPO and Non-Network Providers.  Vision office visit $15.00 co pay at a Kaiser facility.  There is a $250 - $500 annual deductible depending on the provider you choose with an out of the pocket maximum of $3500.00.   Pharmacy co-payments are $15 if using Kaiser or by mail maintenance prescriptions available at 2 co pays for a 90 day supply.  $25 co pay if using FirstHealth Network or Non-Network Providers.  If a generic is available and brand name is chosen, member will pay cost difference, in addition to co pay.

KIPP Dental
Each insured person will have a $25.00 calendar year deductible. The calendar year maximum is $1000.00 for each member covered. Preventive services are paid at 100% of usual and customary charges.

Willamette Dental
This plan is designed to allow insured members to use all Willamette Dental facilities in Oregon. The co-payment per visit is $15. There is no annual maximum paid benefit. Orthodontic benefits are provided on this dental plan.

Life Insurance
Mirabella Portland (MBP) provides Regence Life and Health a comprehensive life-insurance program to benefits-eligible employees.  This benefit includes basic life insurance with accidental death and dismemberment for employees as well as optional life insurance that employees may purchase for themselves and/or their eligible dependents. In addition, the life-insurance program provides valuable added features and services such as portability, guarantee issue, and the accelerated death benefits.

Flexible Spending Reimbursement Accounts
Reimbursement accounts allow you to use pre-tax payroll dollars to pay for eligible premium contributions, dependent day care and Health Care Reimbursement expenses. By using pre-tax dollars for these expenses, it reduces overall tax liability and saves you money. This benefit follows the plan year and money not used will be forfeited.

Voluntary Products
Through Colonial, you may elect to purchase Short Term Disability, Medical Bridge, Accident and Cancer insurance.  Prices will vary based on individual factors.
